One Plan for Two Programs
Some Plainfield residents qualify for Medicare because of age or disability and for NJ FamilyCare, New Jersey's Medicaid program, because of income and assets. People with both are called dual eligible, and they have plan options built specifically for them.
A Dual Eligible Special Needs Plan, or D-SNP, is a type of Medicare Advantage plan that works alongside Medicaid. In New Jersey, many of these plans are fully integrated, meaning one company manages both your Medicare and your NJ FamilyCare benefits under one card.
We explain how each plan we offer coordinates the two programs, check your doctors and medicines, and hold the conversation in English or Spanish.

We do not offer every plan available in your area. Any information we provide is limited to those plans we do offer in your area. Please contact Medicare.gov or 1-800-MEDICARE to get information on all of your options.
How Coordination Works
Medicare pays first for covered hospital, medical and drug costs. NJ FamilyCare then helps with what remains, such as Medicare premiums and cost sharing, and may cover services Medicare does not, such as certain long-term services and supports.
In an integrated plan, a care coordinator helps arrange appointments, equipment and home support, and you deal with one member services line instead of two. Many D-SNPs also include dental, vision, hearing, transportation or over-the-counter allowances, though benefits vary by plan.
- You must keep your NJ FamilyCare eligibility to stay enrolled
- Most plans charge little or nothing in premiums for full dual eligible members
- Prescriptions are covered, and Extra Help usually applies automatically
- You keep your Medicare rights, including the right to appeal
Checking Doctors and Services
We start with the people and places you rely on: your primary doctor, specialists, home care agency, pharmacy and any medical supplies. In Plainfield, that list often includes Neighborhood Health Services Corporation on Myrtle Avenue, a federally qualified health center that serves patients in English and Spanish.
We search each plan's directory for those names and ask what changes if one is missing. If you receive home care or personal care services today, we look at how a plan would continue them.
When You Can Change Plans
Dual eligible members have more flexibility than most. Beyond the fall Open Enrollment Period, current rules provide a monthly special enrollment opportunity for many dual eligible people to join an integrated plan or move to Original Medicare with a stand-alone drug plan.
Rules have changed in recent years, so we confirm which options apply to you before any change. If your NJ FamilyCare renewal is coming up, finish it on time, because losing Medicaid can affect your D-SNP enrollment.
